It has been shown that the rates of capsular contraction are much lower when the breast implant is placed below the muscle. Also, with the implant below the muscle, there is that extra layer of tissue between the visible part of the breast and the implant, therefore making it more difficult to be detected by touch and maintain the natural feeling of the breast. The breast will look more natural with this method as you will not be able to visibly see the edges of the implant.

The technical goal of saline-implant technology was a physically less invasive surgical technique for emplacing an empty breast implant device through a smaller surgical incision.[89] In surgical praxis, after having emplaced the empty breast implants to the implant pockets, the plastic surgeon then filled each device with saline solution, and, because the required insertion-incisions are short and small, the resultant incision-scars will be smaller and shorter than the surgical scars usual to the long incisions required for inserting pre-filled, silicone-gel implants.

A breast lift raises and firms the breasts by removing excess skin and tightening the surrounding tissue to reshape and support the new breast contour. Sometimes the areola becomes enlarged over time, and a breast lift can reduce this as well.
In 2006, for the Inamed Corporation and for the Mentor Corporation, the U.S. Food and Drug Administration lifted its restrictions against using silicone-gel breast implants for breast reconstruction and for augmentation mammoplasty. Yet, the approval was conditional upon accepting FDA monitoring, the completion of 10-year-mark studies of the women who already had the breast implants, and the completion of a second, 10-year-mark study of the safety of the breast implants in 40,000 other women.[119] The FDA warned the public that breast implants do carry medical risks, and recommended that women who undergo breast augmentation should periodically undergo MRI examinations to screen for signs of either shell rupture or of filler leakage, or both conditions; and ordered that breast surgery patients be provided with detailed, informational brochures explaining the medical risks of using silicone-gel breast implants.[113]

There are cases where some patients are wanting a very unnatural or obviously augmented look. Some patients who are athletes or weight lifters, may want to ensure that they are maintaining 100% of their power and strength, therefore if the breast implant is placed over the muscle it allows them to maintain their ability to bench press at their highest weight. Furthermore, there are some patients which due to their anatomy, it is required to place the implant over the muscle. For example, if they had a previous breast augmentation where the breast implants was placed under the muscle that left significant scarring or if they had a severe capsular contraction, it would be recommended that they switch the location of the implant. Another benefit to having the breast implant placed about the muscle is that it can offer a more defined round shape if the patient’s breast has tissue that takes on a different shape.
Healing from a breast implant surgery is not a set timeline. Each patient is different and their body heals at different rates. The actual incision made on the breast will typically heal in about 3-5 days. There will also be absorbable sutures that typically can be removed after 45 days. There will also be bruising present after the surgery, and it can take about one to two weeks to completely disappear. Most pain felt from this surgery is from the muscle being stretched. Generally the patient will be prescribed a pain medication, and depending on their situation their Doctor may also prescribe a muscle relaxant to help the muscle adapt to the new implant. Most patients have reported that after three to four days the pain has significantly subsided and they are able to resume normal activities, such as, driving. The Doctor will also be able to show you various exercises that will help the muscle stretch and assist the implants with settling not the new space more quickly.

Sun tanning damages your skin. It leads to wrinkling, discoloration, and superficial growths. It can also lead  to serious skin cancer. For these reasons, I recommend you use sun block with an SPF number of 15 or greater at all times. A physical broad-spectrum sun block works best. If you are exposed to the sun, I recommend you protect your breasts for six weeks with clothing such as a T-shirt or pareo. You must carefully protect any scars from sun exposure with clothing, tape or sun block for six months.
Round breast implants with a smooth surface are currently used for the majority of breast augmentations in the United States and Canada. These implants tend to create a fuller appearance in the upper portion of the breasts and more distinct cleavage when placed over the muscle (subglandular implant placement). When placed under the muscle (subpectoral and submuscular implant placements), round implants create a very natural shape.
The current lifetime risk of BIA-ALCL in the U.S. is unknown, but estimates have ranged between estimated to be between 1 in 70,000 to 1 in 500,000 women with breast implants according to MD Anderson.[73] Certain geographic locations have demonstrated variable risks. For instance, a December 2016 update from the Therapeutic Goods Administration of Australia and New Zealand reported a risk of 1:1,000 to 1:10,000 for textured implants.".[71] To date (2017), there has not been a case of BIAL reported where the patient had only implantation of smooth shell breast implants or a textured tissue expander that was exchanged for a smooth implant. The paucity of cases reported in Asian populations has raised the possibility that there may be a range of genetic susceptibility to the phenomena, or alternatively merely reflect differences in how cases are identified and reported.
This average total, according to the 2016 statistics from the American Society of Plastic Surgeons, is based on the surgeon’s fee only and does not include the cost of anesthesia, facilities, and materials (stitches, bandages, drapes, etc.). Gently get out of bed as soon as possible after your surgery. When you get up, sit with your legs hanging over the edge of the bed or chair for a few minutes before standing. This will help avoid problems with dizziness, light-headedness and fainting. Do not use your arms or upper body to push yourself out of a bed or chair. You may roll to your side and then sit up or stand up. Have an adult assist you the bathroom the first few times. Always get out of bed to go to the bathroom. Begin to walk as much as possible as soon as you can after surgery. Do take it easy the first few days. Do not exert yourself in any strenuous activity. Avoid strenuous activities that involve arm movement such as raising your arms over your head and lifting with your arms. A balance of rest and reduced activity will speed up your recovery.

Drink only liquids for the first few hours after surgery. Advance slowly to small, light meals. If you experience  nausea or vomiting more than two times, please call me so I can prescribe something to reduce this.  Excessive vomiting and retching can result in bleeding beneath the breast and/or around the implant (if  breast enlargement was done) that will require more surgery.
Despite their differences, most patients who have these procedures to attain their specific goals are highly satisfied with their results. Both procedures are relatively easy to recover from and require 2 to 4 weeks of downtime. Even though both procedures address different issues, it is not unusual for women to encounter both excess breast tissue and sagging. Do not wear a bra for the first week after breast lift or breast reduction surgery. This will reduce your postoperative pain and reduce the risk of shifting the position of the implants after breast enlargement surgery. You may wear any type of undergarment you choose after the first week provided it does not give you pain. It is not necessary to wear a bra at all times.
Both saline-filled breast implants and silicone-filled implants have an outer shell composed of silicone elastomer. This shell is basically a flexible envelope that contains the implant filling. In the case of some anatomically shaped implants, the shell also gives the implants shape. Some models of implants have a "double lumen." This is an elastomer envelope inside of another elastomer envelope (sort of like double-bagging your groceries) which may reduce the risk of implant rupture.
