Structured implants were approved by the FDA and Health Canada in 2014 as a third form of breast implant.[97] Structured implants incorporate both saline and silicone gel implant technology. The filler is only saline solution in case of rupture and has a natural feel like silicone gel implants.[98] The implant uses an internal structure which consists of a series of nested shells that support the upper pole with the two lumen being filled with only saline. The implant is inserted empty and then filled once in place which requires less of an incision than pre-filled implants.[97] If one of the lumen of the structured implant ruptures, it leaks and empties. The other lumen remain intact and the implant only partially deflates, allowing for ease of explant and replacement.[97]

It’s very common for patients to combine their breast reduction surgery with a breast lift to achieve more comprehensive breast enhancement results.

How long you are off work depends on your occupation. If you do clerical work (i.e. stockbroker, teacher, or programmer), you can return to work when you feel up to it. This usually takes about two to three days. Do not go to work for three weeks if you do manual labor (i.e. entertainer, truck driver or personal trainer). Regardless of your employment, do not lift anything weighing more than five pounds for three weeks.

Healing from a breast implant surgery is not a set timeline. Each patient is different and their body heals at different rates. The actual incision made on the breast will typically heal in about 3-5 days. There will also be absorbable sutures that typically can be removed after 45 days. There will also be bruising present after the surgery, and it can take about one to two weeks to completely disappear. Most pain felt from this surgery is from the muscle being stretched. Generally the patient will be prescribed a pain medication, and depending on their situation their Doctor may also prescribe a muscle relaxant to help the muscle adapt to the new implant. Most patients have reported that after three to four days the pain has significantly subsided and they are able to resume normal activities, such as, driving. The Doctor will also be able to show you various exercises that will help the muscle stretch and assist the implants with settling not the new space more quickly.

For my patients, I also provide a post-operative packet. This includes arnica and other ointments, gauze and tape, an ice pack, and nasal decongestant spray. If your surgeon does not provide these items to you before or after your surgery, they are all helpful during the healing process. I encourage my patients to get anti-nausea medication for the first few days following surgery as well.

Patients go to other countries to have rhinoplasty and other surgery performed at a lower cost. Contoured implants, also called anatomical or teardrop-shaped implants, are shaped like a natural breast and create a sloped shape when placed over the chest muscles. Round breast implants have that, well, "round" Victoria's Secret or Playboy model shape. Contoured implants may flip over if the surgeon does not create the pocket correctly, resulting in a misshapen breast. Not true with round implants. If a round breast implant flips, it still looks the same. Your decision on implant shape should be based on how you want your new breasts to look.
Another option is to consider getting your breast implants at a teaching hospital from a learning resident. You won’t get the delicate skill of an experienced, board-certified surgeon, but teaching hospital residents are “assisted by established, experienced, private attending surgeons,” says Beverly Hills plastic surgeon Dr. Robin T.W. Functional breast-feeding difficulties arise if the surgeon cut the milk ducts or the major nerves innervating the breast, or if the milk glands were otherwise damaged. Milk duct and nerve damage are more common if the incisions cut tissue near the nipple. The milk glands are most likely to be affected by subglandular implants (under the gland), and by large-sized breast implants, which pinch the lactiferous ducts and impede milk flow. Small-sized breast implants, and submuscular implantation, cause fewer breast-function problems; however, it is impossible to predict whether a woman who undergoes breast augmentation will be able to successfully breast feed since some women are able to breast-feed after periareolar incisions and subglandular placement and some are not able to after augmentation using submuscular and other types of surgical incisions.[101]

1998 Germany Federal Institute for Medicine and Medical Products Reported that "silicone breast implants neither cause auto-immune diseases nor rheumatic diseases and have no disadvantageous effects on pregnancy, breast-feeding capability, or the health of children who are breast-fed. There is no scientific evidence for the existence of silicone allergy, silicone poisoning, atypical silicone diseases or a new silicone disease."[32]


Transumbilical: a trans-umbilical breast augmentation (TUBA) is a less common implant-device emplacement technique wherein the incision is at the umbilicus (navel), and the dissection tunnels superiorly, up towards the bust. The TUBA approach allows emplacing the breast implants without producing visible scars upon the breast proper; but makes appropriate dissection and device-emplacement more technically difficult. A TUBA procedure is performed bluntly—without the endoscope's visual assistance—and is not appropriate for emplacing (pre-filled) silicone-gel implants, because of the great potential for damaging the elastomer silicone shell of the breast implant during its manual insertion through the short (~2.0 cm) incision at the navel, and because pre-filled silicone gel implants are incompressible, and cannot be inserted through so small an incision.[75]
