The first step toward finding out if you are a candidate is a thorough consultation with a board-certified plastic surgeon who has extensive experience performing breast augmentation. He or she will examine your breasts, your skin tone and the rest of your anatomy to help determine if you are an appropriate candidate. You also may have the option of viewing plastic surgery photos from past patients to better gauge expectations. Other factors, including breast augmentation cost and whether your lifestyle and commitments will allow you to take enough time off to recuperate properly, also play a role in determining your candidacy.
It has been shown that the rates of capsular contraction are much lower when the breast implant is placed below the muscle. Also, with the implant below the muscle, there is that extra layer of tissue between the visible part of the breast and the implant, therefore making it more difficult to be detected by touch and maintain the natural feeling of the breast. The breast will look more natural with this method as you will not be able to visibly see the edges of the implant.
Fat transfer breast augmentation has a few important limitations. First, fat transfer is typically limited to approximately one cup size increase in breast size. Second, the transferred fat may be absorbed again by the body. Third, it is more expensive because of the extra time involved in harvesting and processing donor fat as well as possible need for a special vacuum bra device. This option is best reserved for women who desire a modest increase in size without using an implant.
Structured implants were approved by the FDA and Health Canada in 2014 as a third form of breast implant.[97] Structured implants incorporate both saline and silicone gel implant technology. The filler is only saline solution in case of rupture and has a natural feel like silicone gel implants.[98] The implant uses an internal structure which consists of a series of nested shells that support the upper pole with the two lumen being filled with only saline. The implant is inserted empty and then filled once in place which requires less of an incision than pre-filled implants.[97] If one of the lumen of the structured implant ruptures, it leaks and empties. The other lumen remain intact and the implant only partially deflates, allowing for ease of explant and replacement.[97]
Round breast implants with a smooth surface are currently used for the majority of breast augmentations in the United States and Canada. These implants tend to create a fuller appearance in the upper portion of the breasts and more distinct cleavage when placed over the muscle (subglandular implant placement). When placed under the muscle (subpectoral and submuscular implant placements), round implants create a very natural shape.
Placing the breast implants on top of the muscle, which has been the traditional procedure, can result in a slightly higher risk of contraction. This issue is a cosmetic problem, rather than a medical or health issue. Gel/Silicone implants have been placed both above and below the muscle without a significant difference in the rates of having contraction.

Round breast implants come in a wide array of sizes to accommodate women with different body types. They are also available with a moderate or high profile, which is the amount of projection of the breast profile from back to front. Most round implants have a smooth surface because they can rotate after placement without causing any change in appearance or distorting the breast shape. However, they are available with a textured surface as well.
Body type is a major factor you need to consider before going for breast augmentation. The wrong size of breasts for a particular body type can create many complications like neck and back pain. If your body is small in size and your bone structure is thin, don’t go for huge sized implants. Opt for the size that will make your body look proportional. Similarly if you have a heavy body, you might have to go for larger breast implants.
Since experienced surgeons are aware of these issues with saline breast implants and their need for eventual replacement, they opt to place them under the chest muscle. The chest muscle works as an extra layer of tissue over the implant, which makes for a smoother transition from the chest wall to the implant. The finished product is a more seamless transition versus a more visible and abrupt change when the implant is not placed below the pectoral muscle. As for gel breast implants, they can also be safely placed below the pretorial muscle if that is a viable option for the patient since replacement and wrinkling is less common with this type of implant.
