Silicone implant rupture can be evaluated using magnetic resonance imaging; from the long-term MRI data for single-lumen breast implants, the European literature about second generation silicone-gel breast implants (1970s design), reported silent device-rupture rates of 8–15 per cent at 10-years post-implantation (15–30% of the patients).
Subglandular: the breast implant is emplaced to the retromammary space, between the breast tissue (the mammary gland) and the pectoralis major muscle (major muscle of the chest), which most approximates the plane of normal breast tissue, and affords the most aesthetic results. Yet, in women with thin pectoral soft-tissue, the subglandular position is likelier to show the ripples and wrinkles of the underlying implant. Moreover, the capsular contracture incidence rate is slightly greater with subglandular implantation.
Of course, the breast reduction vs. breast lift question is not really cut and dry because every woman is unique. You may be unsure of what volume you would like with the breasts in a more favorable position. Breasts may be different sizes (asymmetric). The position of the nipple may be out of proportion to the amount of breast sag. Breasts may be ‘deflated’ after pregnancy. You may have heard that breast implants are the only reliable way to lift breasts. These questions can easily be addressed at the time of your consult.
As with any sort of surgery there are always risks involved. The two most common risks for breast implant surgery are bleeding and infection. In general, the risk of bleeding is very low, and if it does happen, it will typically happen within the first 24 hours. Infection on the other hand, can take up to one to two weeks to before it will show itself. However, we take every possible precaution such as; using sterile equipment, sterile gowns, gloves, masks and many other safety precautions. The risk of infection is typically anywhere from two to three percent. So yes, there are risks and they can happen but they are very rare. One other risk that needs to be mentioned is the risk of using anesthesia. So yes, it can happen, but it’s exceedingly rare.

Breast implants have gotten a bad reputation from the past and the fact that they had been known to rupture. The modern-day breast implants are very well made, and it’s almost impossible to cause them to rupture. This doesn’t mean that it can never happen, but it’s exceedingly rare. The rippling and wrinkling however, is a more difficult problem to address. This tends to me more common in the saline implants than with the silicone implants due to the filling. The saline implant is just water which is less dense then the silicone gel. The gel is denser and more closely resembles the actual breast tissue. Even when if the implant is placed below the muscle, it only covers about a half to two-thirds of the implant. Therefore, along the sides or underneath the implant you may see some wrinkling or rippling, however, depending on the size of implant used it may not even be noticeable. The larger the implant used the more visible any wrinkling or rippling could become. Typically, silicone implants are recommended because the risk of any wrinkling or rippling is lower and the implant feels more natural.
The first step in the breast implant surgery process is a consultation with a board-certified plastic surgeon who has extensive experience performing various types of breast surgery. During this meeting, the surgeon will perform an examination of your breast tissue, discuss your goals for surgery and tell you what you need to know about breast implants. Based on his or her examination, the surgeon will determine whether you are a candidate for surgery.
If you have considerable sagging, pendulous breasts, an anchor lift, which allows a cosmetic surgeon to remove a significant amount of excess skin and sagging tissues, may yield the best results. This technique involves 3 incisions: one around the edge of the areola, one vertically from the bottom of the areola to the breast crease, and one along the inframammary fold, hidden in the breast crease. Your cosmetic surgeon may also use this technique if you are having a breast reduction with lift. While the anchor lift comes with some visible scarring, these typically will fade significantly with proper care, and are easily hidden by a bikini top.
Generally, you can go back to working out two to three weeks after breast lift or breast reduction surgery. This depends on how you feel. Do not lift anything that weighs more than five pounds for three weeks. Avoid contact sports for six weeks. If you had breast enlargement with a breast lift, hereafter avoid all exercises which isolate your pectoralis muscles as these can shift the implant toward you armpit. Workouts must stop if you experience discomfort in your breasts or chest. A balance of rest and reduced activity will speed up your recovery.
Many different types of breast lifts are now available in conjunction with a breast reduction, including lifts just around the areola (periareolar), vertical/lollipop mastopexies (incision around the areola and down to the breast fold, and full breast lifts with an anchor incision. For patients who have very loose skin in the armpit or back, axillary or bra lifts are also often combined with breast reductions.
Fat transfer breast augmentation has a few important limitations. First, fat transfer is typically limited to approximately one cup size increase in breast size. Second, the transferred fat may be absorbed again by the body. Third, it is more expensive because of the extra time involved in harvesting and processing donor fat as well as possible need for a special vacuum bra device. This option is best reserved for women who desire a modest increase in size without using an implant.
The study Safety and Effectiveness of Mentor’s MemoryGel Implants at 6 Years (2009), which was a branch study of the U.S. FDA's core clinical trials for primary breast augmentation surgery patients, reported low device-rupture rates of 1.1 per cent at 6-years post-implantation. The first series of MRI evaluations of the silicone breast implants with thick filler-gel reported a device-rupture rate of 1 percent, or less, at the median 6-year device-age. Statistically, the manual examination (palpation) of the woman is inadequate for accurately evaluating if a breast implant has ruptured. The study, The Diagnosis of Silicone Breast implant Rupture: Clinical Findings Compared with Findings at Magnetic Resonance Imaging (2005), reported that, in asymptomatic patients, only 30 per cent of the ruptured breast implants are accurately palpated and detected by an experienced plastic surgeon, whereas MRI examinations accurately detected 86 per cent of breast implant ruptures. Therefore, the U.S. FDA recommended scheduled MRI examinations, as silent-rupture screenings, beginning at the 3-year-mark post-implantation, and then every two years, thereafter. Nonetheless, beyond the U.S., the medical establishments of other nations have not endorsed routine MRI screening, and, in its stead, proposed that such a radiologic examination be reserved for two purposes: (i) for the woman with a suspected breast implant rupture; and (ii) for the confirmation of mammographic and ultrasonic studies that indicate the presence of a ruptured breast implant.
