Subglandular: the breast implant is emplaced to the retromammary space, between the breast tissue (the mammary gland) and the pectoralis major muscle (major muscle of the chest), which most approximates the plane of normal breast tissue, and affords the most aesthetic results. Yet, in women with thin pectoral soft-tissue, the subglandular position is likelier to show the ripples and wrinkles of the underlying implant. Moreover, the capsular contracture incidence rate is slightly greater with subglandular implantation.
The ASPS and the Plastic Surgery Foundation (PSF) have partnered with the FDA to study this condition and in doing so created the Patient Registry and Outcomes For breast Implants and anaplastic large cell Lymphoma Etiology and epidemiology (PROFILE). The United States FDA strongly encourages all physicians to report cases to PROFILE in an effort to better understand the role of breast implants in ALCL and the management of this disease.
Gently get out of bed as soon as possible after your surgery. When you get up, sit with your legs hanging over the edge of the bed or chair for a few minutes before standing. This will help avoid problems with dizziness, light-headedness and fainting. Do not use your arms or upper body to push yourself out of a bed or chair. You may roll to your side and then sit up or stand up. Have an adult assist you the bathroom the first few times. Always get out of bed to go to the bathroom. Begin to walk as much as possible as soon as you can after surgery. Do take it easy the first few days. Do not exert yourself in any strenuous activity. Avoid strenuous activities that involve arm movement such as raising your arms over your head and lifting with your arms. A balance of rest and reduced activity will speed up your recovery.
There are cases where some patients are wanting a very unnatural or obviously augmented look. This can be created by placing the implant over the muscle, as it allows you to really define the sharp edges of the implant, which in turn will create a very obviously implanted look. Aside from that, the advantages of going over the muscle can come in terms of the fact that you are preserving the strength of the pectoralis muscle. Some patients who are athletes or weight lifters, may want to ensure that they are maintaining 100% of their power and strength, therefore if the breast implant is placed over the muscle it allows them to maintain their ability to bench press at their highest weight. Furthermore, there are some patients which due to their anatomy, it is required to place the implant over the muscle. For example, if they had a previous breast augmentation where the breast implants was placed under the muscle that left significant scarring or if they had a severe capsular contraction, it would be recommended that they switch the location of the implant. Another benefit to having the breast implant placed about the muscle is that it can offer a more defined round shape if the patient’s breast has tissue that takes on a different shape.
Firstly about surgical technique. Breast lift and reduction employ similar surgical technique, it's just that with reduction you're removing more breast tissue (because the biggest aim of a reduction is.... to reduce the size of the breast). Does breast lift remove SOME breast tissue, the answer is yes, but the AIM of the lift is more to restore the youthfulness of the breast. Both women, one wanting a reduction vs a lift need their nipple/areolar complex repositioned "up". Only the woman wanting a reduction need the volume addressed.
The technical goal of saline-implant technology was a physically less invasive surgical technique for emplacing an empty breast implant device through a smaller surgical incision. In surgical praxis, after having emplaced the empty breast implants to the implant pockets, the plastic surgeon then filled each device with saline solution, and, because the required insertion-incisions are short and small, the resultant incision-scars will be smaller and shorter than the surgical scars usual to the long incisions required for inserting pre-filled, silicone-gel implants.
For example, if you have very large breasts, sometimes the only reduction that can be made is to detach the nipples and areolas completely from the underlying tissues. If this is done, you will lose sensation in your nipples. Thus, you must think carefully about what will happen during the surgery – and whether or not you can live with the results.
Complications after breast lifts are relatively rare. The main three are bleeding, infection, and scarring. In the early 1990s, the national health ministries of the listed countries reviewed the pertinent studies for causal links among silicone-gel breast implants and systemic and auto-immune diseases. The collective conclusion is that there is no evidence establishing a causal connection between the implantation of silicone breast implants and either type of disease. The Danish study Long-term Health Status of Danish Women with Silicone Breast Implants (2004) reported that women who had breast implants for an average of 19 years were no more likely to report an excessive number of rheumatic disease symptoms than would the women of the control group. The follow-up study Mortality Rates Among Augmentation Mammoplasty Patients: An Update (2006) reported a decreased standardized mortality ratio and an increased risk of lung cancer death among breast implant patients, than among patients for other types of plastic surgery; the mortality rate differences were attributed to tobacco smoking. The study Mortality Among Canadian Women with Cosmetic Breast Implants (2006), about some 25,000 women with breast implants, reported a 43 per cent lower rate of breast cancer among them than among the general populace, and a lower-than-average risk of cancer.

Many patients return to work within the first week after breast lift surgery, depending on the nature of their jobs, and resume most daily activities after a week or so. You will need to limit exercise other than walking for the first 2-6 weeks after a breast lift; your cosmetic surgeon will provide you with detailed instructions about when it is safe to resume any activity.
For my patients, I also provide a post-operative packet. This includes arnica and other ointments, gauze and tape, an ice pack, and nasal decongestant spray. If your surgeon does not provide these items to you before or after your surgery, they are all helpful during the healing process. I encourage my patients to get anti-nausea medication for the first few days following surgery as well.
The ARTOURA™ Breast Tissue Expander or CONTOUR PROFILE® Breast Tissue Expander can be utilized for breast reconstruction after mastectomy, correction of an underdeveloped breast, scar revision, and tissue defect procedures. The expander is intended for temporary subcutaneous or submuscular implantation and is not intended for use beyond six months. Do not use the ARTOURA Tissue Expander nor CONTOUR PROFILE® Tissue Expander in patients where an MRI may be needed. The device could be moved by the MRI causing pain or displacement, potentially resulting in a revision surgery. The incidence of extrusion of the expander has been shown to increase when the expander has been placed in injured areas.
Generally, you can go back to working out two to three weeks after breast lift or breast reduction surgery. This depends on how you feel. Do not lift anything that weighs more than five pounds for three weeks. Avoid contact sports for six weeks. If you had breast enlargement with a breast lift, hereafter avoid all exercises which isolate your pectoralis muscles as these can shift the implant toward you armpit. Platinum is a catalyst used in the making of silicone implant polymer shells and other silicone devices used in medicine. The literature indicates that small amounts of platinum leaches (leaks) from these implants and is present in the surrounding tissue. The FDA reviewed the available studies from the medical literature on platinum and breast implants in 2002 and concluded there was little evidence suggesting toxicity from platinum in implant patients. The FDA revisited this study and additional literature several years later, reaffirming prior conclusions that platinum catalysts used in implants is likely not ionized and therefore would not represent a risk to women.
Subfascial: the breast implant is emplaced beneath the fascia of the pectoralis major muscle; the subfascial position is a variant of the subglandular position for the breast implant. The technical advantages of the subfascial implant-pocket technique are debated; proponent surgeons report that the layer of fascial tissue provides greater implant coverage and better sustains its position.
In 2000, the FDA approved saline breast implant Premarket Approval Applications (PMA) containing the type and rate data of the local medical complications experienced by the breast surgery patients. "Despite complications experienced by some women, the majority of those women still in the Inamed Corporation and Mentor Corporation studies, after three years, reported being satisfied with their implants." The premarket approvals were granted for breast augmentation, for women at least 18 years old, and for women requiring breast reconstruction.
Constance Chen, MD, board-certified plastic surgeon and breast reconstruction specialist, adds that the FDA is currently looking into fat grafting to see if there’s a risk of breast cancer associated with it. Other nonsurgical augmentations with saline or injectable fillers are extremely dangerous and not recommended, says Lara Devgan, MD, board-certified plastic surgeon and RealSelf Chief Medical Officer. “We do not understand how injectable fillers interact with breast tissue in the long run,” she explains.
In 1999, the Institute of Medicine published the Safety of Silicone Breast Implants (1999) study that reported no evidence that saline-filled and silicone-gel filled breast implant devices caused systemic health problems; that their use posed no new health or safety risks; and that local complications are “the primary safety issue with silicone breast implants”, in distinguishing among routine and local medical complications and systemic health concerns.”

Smooth breast implants are round in shape, come in many sizes and are available with different profiles, which is the amount of projection of the breast silhouette from the chest. The shells of smooth breast implants are slightly thinner than textured implants. Smooth implants are less likely to cause rippling of the implant shell and are less likely to rupture.
Structured implants were approved by the FDA and Health Canada in 2014 as a third form of breast implant. Structured implants incorporate both saline and silicone gel implant technology. The filler is only saline solution in case of rupture and has a natural feel like silicone gel implants. The implant uses an internal structure which consists of a series of nested shells that support the upper pole with the two lumen being filled with only saline. The implant is inserted empty and then filled once in place which requires less of an incision than pre-filled implants. If one of the lumen of the structured implant ruptures, it leaks and empties. The other lumen remain intact and the implant only partially deflates, allowing for ease of explant and replacement.
The last thing and the most important decision you will have to make is selecting the right plastic surgeon to perform your breast augmentation surgery. It is critical that you choose somebody who not only has the experience in these types of surgical procedures but maybe more importantly, the credentials. Here in the United States, the best thing you can do is look for a board-certified plastic surgeon who is not only experienced and credentialed but also has the qualities and qualifications that make you feel confident in their abilities.
However, if a patient underwent surgery in another country, but experiences post-operative complications, he or she will need to pay to travel back to the same destination if they wish to have the same doctor oversee any revisions. Revision surgery performed by a different surgeon is extremely difficult, and thus more expensive. In fact, it can cost 50 percent more than the original surgery. Therefore, it can actually be far more economical to pay for a surgery within the U.S.

Since the mid-1990s, the fifth generation of silicone-gel breast implant is made of a high-strength, highly cohesive silicone gel that mostly eliminates the occurrences of filler leakage (“silicone gel bleed”) and of the migration of the silicone filler from the implant pocket to elsewhere in the woman's body. Placing the breast implants on top of the muscle, which has been the traditional procedure, can result in a slightly higher risk of contraction. This issue is a cosmetic problem, rather than a medical or health issue. Gel/Silicone implants have been placed both above and below the muscle without a significant difference in the rates of having contraction.
